NICE positive for Enhertu for treating HER2-low metastatic or unresectable breast cancer after chemotherapy – Daiichi Sankyo

Written by | 9 Oct 2026 | Healthcare Practice

NICE(UK): Trastuzumab deruxtecan can be used, within its marketing authorisation, as an option to treat HER2-low metastatic or unresectable breast cancer in adults after: i) chemotherapy in the metastatic setting, or ii) recurrence during adjuvant chemotherapy or within 6 months after finishing it. Trastuzumab deruxtecan can only be used if the company provides it according to the commercial arrangement.

Why the committee made these recommendations: HER2‑low is a subgroup of breast cancer previously considered HER2‑negative. People with HER2‑low metastatic or unresectable breast cancer have cancer cells with low amounts of HER2 protein. They are offered treatments for HER2‑negative cancer; the type of treatment depends on whether the cancer is hormone-receptor positive or negative. Trastuzumab deruxtecan is the first licensed treatment for HER2‑low metastatic or unresectable breast cancer, and it specifically targets the HER2 protein on cancer cells. Clinical trial evidence shows that trastuzumab deruxtecan increases how long people have before their cancer gets worse and how long they live compared with chemotherapy. When considering the condition’s severity, and its effect on quality and length of life, the most likely cost-effectiveness estimate is within the range NICE considers an acceptable use of NHS resources.

So, trastuzumab deruxtecan can be used.
